How do I print my membership confirmation?

Log in to your USAV account (you can get there by rolling over the “Membership” tab and choosing “Direct Log In Page”), once you have logged in, click on “My Information” on the left, then you will select the second tab from left “Membership Info”, about half way down the page on the left you will see a small white button that looks like a piece of paper (be sure you choose the line with a “CURRENT” membership-this can be seen in Green on the right. You will then press “Print”

I forgot my username and/or password, what should I do?

- If you just forgot your password, please use the “forgot password feature” on the login screen.
- If you forgot your username or your email has changed, you will need to email or call the office at region@columbiaempirevolleyball.com or 503-644-7468

When are Power League results posted?

We try our very best to have them posted on the Monday evening following the tournament, sometimes this is not possible as some results are not turned in on time to the office. We are working harder each year to get the results posted in a timelier manner. Regional results take a few more days, but should be posted by the Wednesday following the regional tournament.

Where is the list of Clubs/Club Directors?

You can click HERE, or you can navigate to the Juniors page. There will be a list there.

I registered for the wrong club, what do I do?

You don’t need to do anything; you can cross out on your membership confirmation the wrong club and write the correct club, we will automatically change it in the system, once we have received your team paperwork from your club director. If you will be trying out for multiple clubs, you can choose “Undecided” as your club.

How do I pay for my membership?

Your membership dues are included in the cost you pay to your club as part of your club dues, you do not need to send CEVA any money for your membership fee.

How does my team get seeded for Power League?

Each club director or coach will fill out a Power League entry form, these can be found
under “Documents”, we ask that the coaches/club directors be as detailed as possible
on this form so that we can seed properly, we combine the information from this form
with the finishes from last season in both Regionals and Power League and give your
team a preliminary seeding. At qualifier your team has the chance to move up or down
quite a few spots depending on their play that day. Seedings are usually done in mid-
December and posted within a day or so for the first tournament in January.

I want to change clubs, what do I do?

First you need to speak with your current coach/club director, they ultimately have the
choice to release you or not. You must be released from your current club BEFORE you
can speak to another club about future possibilities. In order to release a player the club
director must send an email to the CEVA office with the players name and the statement
to release the player.

A player got injured at a game/practice, what do I do?

An Incident report form must be filled out. You can find these under “Documents”, if
you are at a CEVA hosted tournament, please request that the Tournament Director fill
this out and return it to the office. If you are at a practice your coach must fill this form
out. The parent should be given a copy of the “Medical Claim Form” this is the form the
parent will need in order to make a claim. Note: a claim may only be made if an incident
report form was filled out and sent to the CEVA office. There are directions attached to
the Medical Claim Form, please have the parent read all directions for submittal. The
Medical Claim form can also be found under “Documents”.

I need a Certificate of Insurance

The only way to obtain a Certificate of Insurance is to fill out the form “request for
Certificate of Insurance” this will provide the CEVA office with all the necessary
information to create the certificate, please be SURE to include the facility name,
address, and phone as well as a valid email. Please allow 2-3 business days for the
certificates.

I would like to make a complaint, how do I do this?

You will need to fill out the CEVA Complaint form, you can obtain this by emailing
region@columbiaempirevolleyball.com. Once you receive the complaint form please fill
it out to the best of your ability and return it to the office. The CEVA office can only
investigate complaints that are made with a CEVA Complaint form.

I want to coach, what should I do?

You should contact a club that you are interested in coaching for. If they have openings they will speak with you about what you need to do to become a certified coach. We also have a job openings page on our website HERE. All new coaches must be background screened, fingerprinted (this is different than background screening), register for a membership and be IMPACT certified. In addition, all 1st and 2nd year coaches must attend a Ref/Scoekeeping Clinic each year.
